This is SETMA'S Hospital Care Team which is comprised of 15 Registered Nurses, multiple and Nurse Practitioners and Physicians. They maintain the continuity of care between ambulatory and inpatient care, utilizing the same electronic medical record in all clinics, hospitals, nursing homes and provider homes.
