Public Reporting - PCPI Care Transitions

This Physician Consortium for Performance Improvement data set was only published in June, 2009. When SETMA saw it, we realized that we have been doing all of the elements of the set for a number of years. Because we complete discharge summaries at the time of discharge and in our EMR at the hospital, all 14 data points and all 4 actions are consistently completed on all patients being discharged from the hospital or from the emergency department.
