 The award recognizes practices for improving care by: 
  Using health information technology in every day practiceEmploying population care managementParticipating in data reporting quality initiativesAchieving exemplary performance on identified clinical quality  measures The Centers for Medicare & Medicaid Services (CMS) has  designated these areas as national health care priorities.  The award  recognizes physician practices that are active in quality improvement and  performed well in each of the above categories. About  the award 
  Out of approximately 135  practices that self-nominated, 80 practices were recognized as award winners.TMF Health Quality Institute, the Medicare Quality Improvement  Organization for Texas, established the awards program in partnership with  Texas Medical Association and Texas Osteopathic Medical Association.Physician practices self-nominated in the summer of 2012,  demonstrating their commitment to quality improvement. Over the course of 2012,  they utilized health information technology in every day practice, regularly  used population management methods, participated in a data reporting quality  initiative and performed at or above the 90th percentile on  nationally recognized clinical outcome measures, such as smoking cessation,  blood pressure control and cancer screening.  Specific criteria had to be  met to achieve the award.Practice size was not a factor in determining who received an  award. Award recipients included solo practitioners with one location to large  multi-specialty practice with multiple locations.  Practices could apply  either a single site/location or as a network.Each practice abstracted and submitted data form their EHR with  supporting documentation to meet the requirements for participation in the  award. |
