Subject: Hypertension collaboration with the AMA
Dear colleagues,
Hope all is well .
We are collaborating with the AMA quality improvement folks on a program to help primary care practices improve the control of their hypertensive patients. The AMA has developed a checklist of actions that lead to better BP management and control. The checklist is attached. AMA asked us to get involved since many of the actions on the checklist involve the whole practice team. Our role will be to describe how LEAP sites with benchmark BP performance use their staff and systems to manage BP (and carry out checklist steps), and provide case studies and tools to help other practices.
You may not remember, but you shared performance data with us back when we were selecting the LEAP sites. The data you shared with us seemed to indicate that your clinic provided excellent hypertension care. What is the most recent percentage of your hypertensive patients with BPs 140/90? Would you and other relevant folks from your clinic be willing to talk with us about your management of high blood pressure? We can probably get what we need from a single phone call. Thanks for considering this.
