Letters - PC-MH in Southeast Texas

I spent some time this afternoon (August 12, 2016) looking at the Patient-Centered Medical Home (PC-MH) practices in our region.

SETMA is the only PC-MH practice in the following 10 counties: Jefferson, Orange, Newton, Hardin, Chambers, Liberty, Jasper, Tyler., Angelina, and Polk.

Not only does this prepare us to succeed in MACRA and MIPS payment models, but it prepares each of us to compete our careers with pride and dignity.
