health care quality through exemplary performance on national quality priorities.

The award recognizes practices for improving care by exceptional performance on
clinical quality measures, population care management strategies,
data reporting on quality initiatives and implementation of patient engagement strategies.

These areas have been designated as national healthcare priorities by
the Centers for Medicare and Medicaid Services.
