I am delighted to have the opportunity to serve on the NQF Endocrine Standing Committee.  In addition to the information on my NQF bio, SETMA has NCQA Diabetes (2010-2016) and Heart/Stroke Recognition (2013-2016), and Patient-Centered Medical Home accreditation by NCQA (2010-2016, AAAHC (2010-2014, URAC, survey December, 19-20, 2013, and Joint Commission, survey February 2014.

By way of introduction, the following annotated links to materials on Southeast Texas Medical Associates’ (SETMA) website are offered:

SETMA.com | Presentations | The Importance of Data Analytics in Physician Practice

This presentation was given at the Massachusetts Medical Society Informatics meeting in the spring of 2012.  It addresses the critical role which data analytics has to practicing physicians and explains SETMA’s approach to data analytics.  In 2011, SETMA worked with IBM to define the Physician Analytics Quotient (AQ).

SETMA.com | Letters | The Primary Care Team: Learning from Effective Ambulatory Practices (PCT-LEAP)

This paper was prepared for the Robert Wood Johnson Foundation in response to a request for information about SETMA for participation in the Learning from Exemplar Practices (LEAP), a research project on primary care workforce innovations conducted by the MacColl Institute.  SETMA was selected.  The interest for NQF is the following sections of that report which can be accessed by the following hyperlinks:

SETMA’s Model of Care
SETMA’s Quality Metrics Philosophy
The Limitations of Quality Metrics
Team Approach to Healthcare Delivery

SETMA.com | Public Reporting | Reporting by Type

SETMA began publicly reporting by provider name in 2009.  This month, we complete five years of reporting by provider name on over 250 quality metrics. Below are links to the NQF endorsed metrics which are included in those reports.  I have highlighted the NQF Diabetes Measures which are included below.
