Please contact me with any questions or concerns. I hope that SETMA will continue to work well with our case management department. It seems that we have trouble with communications with some of your physicians. Do you have suggestions for me? I'm happy to contact them in any way that they prefer so as to not disrupt their work flow. Text messages seem easy and effective. Please let me know so our case management efforts can be more efficient and effective.

My group of doctors includes Drs. Pestana, Hoggard, Kushin & Brundage. We work with case management at TMCSET. We basically act as a liaison between the physicians and the case management department. We contact physicians when the plan of care is not well understood or incompletely documented in the chart. We frequently call with questions about discharge plan especially when patients stay beyond their GMLOS.
