Monthly, SETMA’s clinics are closed for half a day, and all SETA providers gather for lectures, discussions and demonstrations on how to improve our clinical performance.  We have worked hard on reducing hospital readmissions for several years and have been successful.  The Institute for Healthcare Improvement – the same organization which gave us the Triple Aim -- has published four sets of materials on Transitions of Care from inpatient status to clinic, post-acute care, skilled nursing and home health. 

SETMA’s Provider training for June will take place on June 26, 2012.  The attached slide deck is an in-depth review of IHI’s Improving Transitions from the Hospital to The Clinical Office Practice to Reduce Avoidable Rehospitalizations.  Subsequent to that, in July, August and September, we will review the other sets.  This slide deck includes an extensive review of IHI’s work, of the tools SETMA has built and of the leverage points we have identified through our work and that of IHI.
